Determine whether the culture bearer is nominated as a Master Artisan or an Emerging Artisan.

Master Artisan Emerging Artisan

1. Proof of Identity *

Scanned digital copy of PSA Birth Certificate or valid government ID (.JPG or .PNG).

2. Bionote of the Nominee *

300–500 words in English or Filipino. Written in Google Docs, Times New Roman, single-spaced, 16pt title, 12pt body, A4 size.

3. Visual Evidence of ICH Transmission (Min. 10 photos) *

Prior photo documentation of workshops, apprenticeships, or community activities with source citations/attributions included.

4. Two (2) Duly Accomplished Endorsement Forms *

Completed by people who know the culture bearer well — a family member, friend, neighbor, co-worker, or peer (DOT-III Template). It’s the LGU’s responsibility to collect these on the nominee’s behalf.

5. Video File (Documentation / Archival Footage & Interview) *

Includes required interview of the culture bearer focused on their practice, max 2 minutes, raw format encouraged.

6. Supporting Documents / Certificates / News Articles *

Scanned certificates from workshops, seminars, competitions won, or screencapped news articles (.JPG/.PNG).

7. Recent Profile Photo of Culture Bearer *

Portrait style, vertical format (formal shot or candid while working), submitted as .JPG or .PNG.

8. 1,500-Word Essay & Landmark Works Dossier *

Must include specific local name in title (e.g., “Ang Pagyabong ng Kulturang ‘Pamandukit’ sa Betis”), name of nominee, category, 2-3 landmark works with details, embedded images, transmission explanation, Times New Roman single-spaced, 16pt title, 12pt body, A4 size.

9. Additional Requirement for Chanting, Traditional Healing, or Ritual Performance (Conditional)

If applicable: Submit a 3-minute video featuring authentic performance demonstration, combining archival/recent footage, with narrated voice-over on cultural value and historical context.
